Pre Admission Booklet (general admissions) – 14 pages (Acrobat file)
This booklet contains instructions to assist you in preparing for your admission and includes the following:
- Information to assist you in completing your forms
- Instructions on what to do before you are admitted
- Information about your account
- Anaesthesia for your Procedure
- Information about Blood Transfusion
- A number of Hospital Policies including
- Personal Information and Privacy for Patients
- Instructions and a map on how to get to the Hospital
Pre Admission Forms to be completed by you
Admission Form – 2 pages (Acrobat file)
This is an administrative form that needs to be completed by you and sent to the Hospital prior to your admission.
Patient History Form – 4 pages (Acrobat file)
This is a clinical information form that needs to be completed by you and sent to the Hospital prior to your admission along with your other forms. Information on this form will be used by your anaesthetist and nursing staff to assess your particular care requirements.
Patients undergoing a day procedure need only complete pages 1-3 (and sign page 4). For patients staying overnight, please complete all pages.
Pre Admission Forms to be completed by your Doctor
Hospital Booking Letter Form – 2 pages (Acrobat file)
This is a booking form that your doctor will complete and fax to the Hospital prior to your admission. Please keep the original and bring it with you when you are admitted to hospital.
Consent to Medical / Surgical Treatment Form – 1 page (Acrobat file)
This is a consent form that your doctor will complete in consultation with you. It must be signed by both parties and sent to the Hospital prior to your admission along with your other forms.
